Very First Initial Appointment - Choose the appropriate new client form to download, print and fill out completely to bring with you to your first appointment. If you do not, please arrive at least 15 minutes early, otherwise arrive 10 minutes early.
Subsequent Appointments - Please use the returning client consent form and arrive 5-10 minutes early.
If the client is a minor, please choose both the new client form and the minor release form for their first appointment and just the minor release form for all subsequent appointments.
